Maybe your pot not having good contact? Did you try rebench few times reassembling everything?
Let's get into could be, that my damaged CPU is the fail...BUT, I don't think, that the CPU is responsible therefore, that the card goes black, adjusting more than 1,625V!
So, let's get into details:
4770K (damaged, only PCIe 8.0 and 3 cores active)
TridentX running 2666 9-11-11-28 1T
Cooling DICE
Pot was assembled many times...the first time I assembled it, was on friday, and there I was able to run 1550 with 1,525V...yesterday, my waterbench was better than any dice bench, so there must have been something wrong.
The soldering was done by der8auer who did that not the first time. It runs quite good, but the 1,625V barrier is not really nice...I need more on thursday, coming 50L LN2!
Also the problem is cb...on friday, there has been no CB...yesterday it doesn't like less than minus 25 and more than minus 12...frida was full pot no prob!
Do you need more information?

I don't need more, but you might

First of all, just to be clear, i never benched GK110 on Haswell.
You seem to be too much focused on temps or voltages, but not actually system running or benchmark result.
Get 1.4V, easy temps, study system, make sure everything runs smooth.
Then bring temps lower, find the limit. Check if your memory working good, that
all stuff is behaving good. We were using TEK9 FAT/SLIM pots for all those Titan benching (few dozen of cards, Titan EPOWERs/780/780classy's) and did not see
any wierd stuff you describing. All these cards had sweet spot temps, every card different. And with 1.5V you can get much higher clocks than you posting here.
And with 1.6V enough be close to some records, that's for sure
